Care1st Health Plan takes the protection of our members PHI/PII seriously. Remittance Advices associated with payments for patient care contain PHI/PII and could put that information at risk. Remittance Advices will be available only through SECURE electronic download.

Attention Ambulatory Surgery Centers:
Please be advised that Care1st Health Plan requires the Ambulatory Surgery Centers to submit Medicare claims using the CMS-1500 claim forms. Claims submitted using form UB-04 are rejected and will need to be re-submitted.

Physical Accessibility Review Survey – DHCS Requirement for Medi-Cal Managed Care and Cal MediConnect provider sites
