Health Education for Providers

The Centers for Medicaid and Medicare Services (CMS) requires that all newly enrolled Medicare members must have a full health risk assessment or a good faith effort to complete one within 90 days of the enrollment date.

Care1st Health Plan sends members a health risk assessment survey to complete within 30 days of the enrollment date. If the member does not return a survey we follow up with at least two call attempts to complete the survey. Care1st will send the primary care physician (PCP) a summary of the health risk assessment for their records. It’s important that you review this information with your member during their next visit.

Care1st is asking its contracted provider offices to also make attempts to contact and schedule an office visit with all new Medicare members. This office visit helps to establish care and for you to understand the members medical history and assess any specific needs.

Care1st has Health Risk Assessment tool that we can send to you upon request. These tools include a risk assessment, functional assessment and progress note for this visit. You can request copies of these tools by contacting Norma Perez at (323) 889-6638, extension 6281.
