About Cal MediConnect

How it Can Help

The Cal MediConnect program's main goals are to make it simpler to get the care and services patients need from both Medicare and Medi-Cal, while also helping them to stay independent in their own homes and communities. With a participating health plan like Care1st, there will be more of the personalized care patients need with greater healthcare savings and less of the "runaround".

Cal MediConnect brings together all Medicare and Medi-Cal benefits under a single health plan. These benefits include:

  • Medical Care
  • Prescription Medications
  • Mental Health and Behavioral Health Care
  • Long-Term Services and Supports (LTSS)
  • Connection to Social Services (Like Meal Delivery)

Both members and providers will benefit from the program's smoother, simpler process.

Care1st's Goals

Care1st Health Plan has a long history of serving dual eligibles. We know how to work with doctors and hospitals as well as the Medicare and Medi-Cal offices to put our members' needs first. As part of Cal MediConnect, we expect to successfully carry out the program's goals:

  • To bring all benefits and access to care together under one health plan
  • To improve our members' health
  • To give our members a voice and a choice when it comes to their care
  • To prevent unnecessary ER visits, hospital stays, tests and prescription interactions
  • To help members live safely at home with access to support services, rather than in institutional care
