Care1st Health Plan Arizona, Inc.
In 1994, a group of practicing physicians and a hospital in southern California came together to create a progressive, service-driven health plan that now serves over 200,000 people in the Los Angeles, San Bernardino and San Diego areas. In 2003, they brought this concept to Arizona and created Care1st Health Plan Arizona, Inc., which presently serves Maricopa and Pima Counties.

Care1st Health Plan is proud to provide coverage to beneficiaries of Arizona's implementation of the Federal Medicaid program, the Arizona Health Care Cost Containment System (AHCCCS), as well as coverage for Care1st members who are enrolled in the Division of Developmental Disabilities (DDD) program.
In order to become a Care1st member, you apply for health coverage through AHCCCS and choose Care1st as your AHCCCS or DDD health plan (individuals who obtain services must meet certain income and other requirements). If you are already enrolled with AHCCCS, you may choose Care1st Health Plan on the anniversary of your enrollment.
