Care1st CARES Programs

Medi-Cal Members

 



Care1st has partnered with Alere, Inc for our asthma program and Healthways, Inc for our Congestive Heart Failure (CHF) and Coronary Artery Disease (CAD) Programs.Care1st automatically identifies patients with these medical problems and refers them to these programs. If you feel you might have any of these medical conditions but are not sure please make an appointment with your doctor. You can self refer to these programs or your doctor can refer you by filling out this form.

Asthma Program

Asthma is a lung disease that narrows the airways. It makes it difficult for you to breathe. It can not be cured, but it can be controlled if it is treated correctly. You can live a normal life. If you are not sure you have asthma contact your doctor and make an appointment.

The more you learn about asthma and the medicines used to treat it, the better you will be able to work with your doctor to control it. In order to help you meet these goals, Care1st Health Plan has an asthma program available to you. This program can teach you all that you need to know so you can be in control of your asthma.

Care1st has partnered with Alere, Inc. to give you additional educational resources.

  • Toll-Free Advice Line - There Nurse Advice Line is staffed by registered nurses that can answer questions related to asthma. Just call 1(800) 655-LUNG, from 7:30 am to 3:30 pm.
  • Smart Living Asthma Guide - This guide helps you understand and manage your asthma
  • Regular Nurse Calls - Alere nurses routinely contact our enrolled asthma members. This call takes about 15-20 minutes every few months.
  • Reports for your Doctor - Alere provides regularly reports so your doctor understands your progress.

Coronary Artery Disease (CAD) and Congestive Heart Failure (CHF) Programs

Care1st Health Plan has partnered with Healthways, Inc. to provide additional resources for Coronary Artery Disease (CAD) and Congestive Heart Failure (CHF). These resources include:

Health Guides & Tools - Throughout the year, you will be sent valuable educational resources to help manage your condition. These materials include:

  • Education about your disease
  • Explain tests and reports
  • Help you keep track of your medications, and
  • Help you create a health management plan.

Healthy Newsletters & Reminders - You may receive newsletters and reminders that will:

  • Update you on the latest breakthrough treatments
  • Remind you about important exams, and
  • Share lifestyle changes that could make a difference in your health, and more.

Support Available Monday - Saturday - Highly trained care team professionals will:

  • Review your health status
  • Discuss your condition
  • Help you with lifestyle changes
  • Contact you once in a while by phone to assist you with the management of your condition, and
  • Answer any questions you have.

If you have any of these conditions someone from the program may call you to talk to you about the program. They will ask to schedule a time to speak with you about your health.
