Care1st Health Plan is an HMO that contracts with the federal government to provide Medicare services and benefits.
Care1st Health Plan serves a specific service area. The service area for the Care1st Medicare Advantage Plan and the Dual Plus
Plan includes: Los Angeles*, Orange*, San Bernardino*, and San Diego Counties, CA. The service area for the Care1st Platinum Plan includes: Los Angeles*
County, CA. You must live in one of these areas to join the plan. You can join Care1st Medicare Advantage Plan if you are entitled to Medicare Part A and
enrolled in Medicare Part B and live in the service area.
*Note: Only parts of these counties are covered. For more information, call Care1st Member Services at 1-800-544-0088 (TTY 1-800-735-2929) Monday through
Friday, 8:00 a.m. to 6:00 p.m. Pacific time.
All Medicare Advantage and Medicare Prescription Drug Plans agree to stay in the program for a full year at a time.
Each year, the plans decide whether to continue for another year. Even if a Medicare Advantage or Medicare Prescription Drug Plan leaves the program,
you will not lose Medicare coverage.
If a plan decides not to continue, it must send you a letter at least 90 days before your coverage will end.
The letter will explain your options for Medicare coverage in your area.
