Health Plan Arizona has surveyed it’s contracted
hospitals to inquire which Patient Safety initiatives
have been implemented and
we are sharing that information with our practitioners.
Of the responding hospitals (30%), all had implemented
the following:
- Universal Protocol for Preventing Wrong Site/Procedure/Person
Surgeries
- Using at least two patient identifiers for
medication or blood product administration, taking
blood samples, or providing
other treatment or procedures.
- Requiring staff to read-back telephone/verbal
orders or critical test results.
- Adoption of the “Do Not Use” acronym
list.
- Tracking and assessing the timeliness of reporting
critical test results and values.
