How do I apply?
Care 1st Health Plan Arizona, Inc. does not enroll you into our
plan. You must first apply for Health coverage through your local
DES office or AHCCCS Administration, or SSA office.
Visit your local DES office and fill out the application for
an appointment. You can also call (602) 542-9935 between 8:00am
and 5:00pm Monday through Friday to have a local DES office near
you mail out an application to your home address. Remember to
speak clearly when giving your name and address.
For KidsCare, free assistance in filling out the application
is available by phone. Call (602) 417-5437 in the Phoenix area
or call 1-877-764-5437 toll free.
You can also visit the AHCCCS or KidsCare websites to download
applications.
What do I bring with me?
Bring any information that will help the caseworker determine
your eligibility for AHCCCS or KidsCare. Bring documents such
as:
• Identification (I.D.)
• Social Security numbers for everyone, or proof that a Social
Security number has been applied for
• Birth certificates for everyone
• Pay check stubs
Bringing all your information will make the eligibility process
faster for you. Once you are eligible, you may then receive health
care coverage under one of Care 1st Health Plan Arizona, Inc. programs.
