What services are covered?
A standard benefit package is offered to all enrollees
of Care1st enrolled in Healthcare Group of
Arizona. Services include:

• Emergency medical services, including emergency transportation

Who is eligible?
If you are a group of two participating employees or greater, you may enroll in the HCG program. If you are a group of one, however, recent legislation has removed HCG’s authority to enroll groups of one. If only one employee of your business is interested in enrolling, we regret to inform you that your business is ineligible at this time.
How can I enroll?

an enrollment packet or
download the forms here.
The enrollment materials that highlight the benefits of the Healthcare Group program and detail the eligibility requirements for small businesses, as well as provide all the forms needed to enroll are located at http://www.hcgaz.com under Prospective Members/Enrollment Packet.
Although all forms, benefit options, Group Service Agreement, and premium rates are available to you in the packet available online, all newly-enrolling groups are required to attend an enrollment workshop to ensure your enrollment will be processed. The workshops will not only highlight the benefits of the plan options, but will explain the plan exclusions. An updated workshop schedule can be found online at www.hcgaz.com/enrollworkshop.pdf. (If you only want to list dates at Care1st, that is okay with us since Brandon will be handling these directly)
If you are unable to attend an enrollment workshop, please contact HCG to schedule an appointment for your group. We may be able to discuss your questions over the phone, by appointment at HCG or have an enrollment associate visit your office.
