Welcome to Care1st Health Plan Arizona, Inc. (Care1st)!

In 1994 a group of practicing physicians and a hospital in southern California came together to create a progressive, service driven health plan that now serves over 200,000 people in the Los Angeles, San Bernardino and San Diego areas. In 2003, they brought this concept to Arizona and created Care1st Health Plan Arizona, Inc. These same physicians lead Care1st today and continue to focus on our mission:

A provider-oriented managed care organization that strives to continuously improve the quality of services rendered to its members and to provide excellent customer service to both members and providers.

Care1st provides coverage to beneficiaries of the Arizona Health Care Cost Containment System (AHCCCS) in Maricopa County, including members enrolled in the Division of Developmental Disabilities (DDD). We have been dedicated to serving families in Arizona since our inception in 2003. We are proud to serve you and your family, and to provide you with the quality health care services that you deserve.

Enrollment

In order to become a Care1st member, you apply for health coverage through AHCCCS. AHCCCS is Arizona's Medicaid agency and it offers health care programs that serve Arizona residents. Individuals that obtain services meet certain income and other requirements. While applying for AHCCCS you can choose Care1st as your AHCCCS or DDD health plan. If you are already enrolled with AHCCCS, you may choose Care1st on the anniversary of your enrollment.
