Care1st Health Plan wants to assure you understand your rights with our credentialing process.

  • You have the right to review your credentialing application, which includes information that may be obtained from an outside source such as state licensing boards. Care1st will limit this review as it pertains to peer-review protected information.
  • You have the right to correct erroneous information. Care1st reviews applications and compares this information with information collected from outside sources. When there are discrepancies Care1st will notify you, but we are not required to reveal the source of information. Care1st will assure that when we notify you of any discrepancy that we include the following:

• The time frame for changes to be made
• The format for submitting corrections
• The contact person
• Confirmation we received the corrections
• You have the right to be informed of your applications status upon request.
